Fingerprinting Appointment Form
Once you are scheduled and have received your assigned appointment, if you need to cancel, you must send an email to:

If you are sick or have a family emergency on the day of your appointment, you should cancel your appointment.

An applicant that fails to show for an appointment may have to wait to re-apply for a new appointment.
