This form is only going to be available this pay period. Thereafter, you'll report your issues via TSR as you would any other tech issue.
To adjust your expectations, please allow for an entire shift before you check to see if the problem persists. Feel free to check sooner, but understand that the MCG MCtime team is working hard to adress everyone's needs.
