This form will generate an email to the assigned Shift Chief for the shift the activity is scheduled on. You will receive a confirmation email.

**NOTES**
1. Both career and LFRD personnel must continue to make requests for apparatus movement through their chain-of-command.
2. REMEMBER activities requests made within three days of the target day must be emailed to the On duty Shift Chief. You should Cc the assigned Shift Chief.
3. Requests for Ladder Testing, Pump testing, Cot Maintenance and other CMF recurring activities continue to go through the Public Safety Logistics Chief.
4. Community Risk Reduction Program requests continue to be processed by the CRR Battalion Chief.
5. Requests that do not come from a recognizable domain (@montgomerycountymd.gov, @kvfd.org, @rvfd.org, etc…) will be discarded.
